Job Summary:

The Violence Prevention Specialist/Care Coordinator (Nurse, MSW, LCSW) will serve several roles for the Comprehensive Hospital Based Adolescent Violence Assessment and Treatment at Children's Hospital Colorado (CHAVAT@CHCO). The Violence Prevention Specialist/Care Coordinator (VPS-CC) will be primarily responsible for the coordination of patients interfacing with the CHAVAT, including reviewing charts for eligibility for the program, providing outreach to families and coordinating intakes. The VPS-CC will participate in clinical care, including helping with assessments, and will provide brief clinic based motivational interviewing interventions for youth at risk for- are having already perpetrated- violence. MI focused interventions will focus on ways to reduce both violence perpetration, gun carrying and gun violence in the outpatient setting. The VPS-CC will communicate treatment plans to the other behavioral health specialists involved in the CHAVAT as well as coordinate appointments with youth and their families. The VPS-CC will assist in coordinating wrap around services, setting up appointments between youth identified in the emergency department or hospital setting for outpatient management. We anticipate the VPS-CC to participate in the care of approximately 100 clients annually. This is a grant funded position.

Key Responsibilities:

  • Participate in the care of approximately 100 clients annually.
  • Other activities as deemed necessary by supervisor.

Onsite - this role is expected to work onsite and is located in Aurora, CO.

Qualifications:

Minimum Qualifications:

Applicants must meet minimum qualifications at the time of hire.
  • RN, MSW, LSW, LCSW
  • Master's Degree
  • Clinical experience working with adolescents and families

Preferred Qualifications:
  • Ideally but not limited to at least 3 years' experience working with at risk adolescents and young adults (substance use, juvenile justice, violence prevention).
  • Proficient in Motivational Interviewing skills
  • Experience with trauma informed care.

Knowledge, Skills and Abilities:
  • Counseling skills
  • Efficient with organizational skills, basic computer skills
  • Excellent communication skills
  • Ability to work with a team of health care providers

Anticipated Pay Range:

The starting salary range (or hiring range) for this position has been established as HIRING RANGE:

Instructor: $60,000 - $75,000 Based on 0.70 FTE
