Job Summary:
The CU Medicine Geriatric Medicine Clinic located in Kavod Senior Life is seeking a compassionate licensed clinical social worker for care management and integrated behavioral health support to join our multidisciplinary team in a geriatric primary care clinic. Kavod Senior Life is a U.S. Department of Housing and Urban Development (HUD)-subsidized, independent and assisted living facility serving low-income seniors with complex medical and psychosocial needs. Our clinic is a unique academic-community partnership and innovative care delivery model providing patient-centered primary care to residents of Kavod as well as patients from the community, with a focus on improving the overall wellbeing of older adults through a collaborative, team-based approach.
This integrated care management and behavioral health position will provide comprehensive care coordination, resource navigation, and evidenced-based brief interventions to older adults and their families. This position will work in close collaboration with the medical team, including 3 geriatricians (2 MDs and 1 NP), a clinical pharmacist, a lead nurse, and a medical assistant. This position will also facilitate care coordination with staff at Kavod, including the director of health and wellness, director of assisted living, and multiple care managers.
This is a 1.0 FTE position with 40-hour work week, including 4 clinical days, and 1 administrative day (schedule negotiable). Administrative days are used to actively participate in Division of Geriatrics meetings, clinical care conferences, and university service/committee work. There may be opportunity for partial remote work, depending on patient care and clinic needs.
Key Responsibilities:
Care Coordination & Case Management
- Assess the psychosocial needs of geriatric patients and families dealing with chronic illness, aging, and end-of-life issues.
- Develop and implement individualized care plans in collaboration with multidisciplinary team.
- Identify and connect patients with appropriate community resources, including transportation, housing, and financial assistance.
- Assist patients in applying for public benefits, including Medicaid, Medicare, and long-term care services.
- Advocate for patients and address barriers to care, including socioeconomic factors and health literacy issues.
- Serves as a liaison between the patient, healthcare providers, Kavod Senior Life, and other community agencies.
Behavioral Health & Clinical Care
- Uses an integrated care model to briefly assess, diagnose, and treat behavioral, mental, and health conditions using evidence-based and solution-focused interventions.
- Offers crisis intervention and support for patients experiencing acute mental health issues, including conducting suicide risk assessment, connecting patients with appropriate specialty mental health resources, and communicating with community specialty mental health providers as needed.
- Coordinates referrals to specialty mental health services when indicated, ensuring continuity of care.
- Provides clinical consultation for behavioral health-related problems to geriatric medicine providers.
- Contributes to the development of behavioral health protocols and workflows to enhance the integration of mental health care in the primary care setting.
- Participate in QI projects and facilitates group visits as needed.
Work Location: This role is expected to work onsite and is located in Aurora, CO.

Qualifications:
Minimum Qualifications:
Applicants must meet minimum qualifications at the time of hire.
- Master's degree in social work from an accredited institution.
- Current and unrestricted Colorado state licensure as a LCSW, or the ability to obtain one prior to starting the position.
Preferred Qualifications:
- Ideal candidates will have at least 5 years of experience in care management and integrated behavioral health in primary care or related healthcare settings.
- Expertise in evidence-based behavioral interventions and psychotherapies (e.g., motivational interviewing, cognitive-behavioral therapy, acceptance and commitment therapy, etc.).
- Experience working with older adults and knowledge of common behavioral and physical health issues among older adults (e.g., chronic disease management, sleep disruption, depression/anxiety, cognitive decline, grief, caregiver stress, etc.).
Knowledge, Skills and Abilities:
- Clinical skills conducting brief and targeted assessments to provide evidence-based brief interventions, health and behavior change, and triage for appropriate level of care.
- A growth mindset that supports flexibility and adaptability in an evolving healthcare environment.
- Works well with diverse patient populations.
- Works effectively both as part of a team and independently.
- Ability to establish and maintain effective working relationships with clinic providers and staff.

Anticipated Pay Range:
The starting salary range (or hiring range) for this position has been established as HIRING RANGE:
$65,000-$80,000 for a 1.0FTE position
